Approach and Management of Shoulder Instability

Shoulder instability encompasses a spectrum from symptomatic laxity to recurrent dislocation, commonly affecting young and active individuals. This presentation, delivered from an orthopaedic surgeon’s perspective, is designed to provide healthcare professionals with a clear and practical framework for assessment, referral, and rehabilitation.

The session will begin with a review of glenohumeral anatomy and biomechanics, highlighting the roles of static stabilisers (labrum, capsule, ligaments) and dynamic stabilisers (rotator cuff and scapular muscles). Differentiating between traumatic structural instability and atraumatic or multidirectional instability is essential for guiding management.

A structured clinical approach will be outlined, including key subjective findings, physical examination tests, and indications for imaging. Common pathologies such as Bankart lesions and Hill-Sachs defects will be discussed in the context of clinical decision making, particularly in identifying patients suitable for conservative management versus those requiring surgical referral. The assessment and management of posterior shoulder instability and multi-directional instability and specific management of the throwing athlete with regards to SLAP tears will be covered as well.

Surgical options, including arthroscopic stabilisation and bone augmentation procedures will be briefly covered, with emphasis on indications and expected outcomes. Importantly, the implications of these procedures on rehabilitation protocols, precautions, and timelines will be highlighted.

The presentation will conclude with key rehabilitation principles, focusing on restoring dynamic stability, scapular control, and safe return to sport. Emphasis will be placed on optimising outcomes through close collaboration between surgeon and healthcare professionals across the continuum of care.

About the speaker

Dr Puah Ken Lee

MBBS (S’pore), MRCSEd, M.Med (Ortho Surg), FRCSEd (Orth), FAMS

Dr Puah Ken Lee is a Consultant Orthopaedic Surgeon with a special interest in shoulder, elbow and sports injuries. He is currently Chair of the Chapter of Orthopaedic Surgeons at the Academy of Medicine Singapore and Chair of the Singapore Shoulder & Elbow Society. He is also a Corresponding Member of the American Shoulder and Elbow Surgeons, reflecting international recognition in shoulder surgery.

Dr Puah graduated from the Yong Loo Lin School of Medicine, National University of Singapore and completed his orthopaedic training under SingHealth. He previously served as a Consultant with the Sports Service at Singapore General Hospital.

In his practice, Dr Puah focuses on clear diagnosis, thoughtful treatment planning and helping patients return confidently to daily activities and sport. He believes that good care begins with listening, explaining options clearly, and tailoring treatment to each individual’s goals.
